已有明確範圍的工程或研究觀察未達預期、暴露風險或顯示不可接受結果；不自動等於臨床傷害或 adverse event。
不要把 absence of evidence 誤寫成 negative clinical result。本 registry 的 negative entries 是明確限縮的 engineering findings；目前沒有聲稱 repo 內存在 clinical adverse outcome、patient harm、production incident 或 regulatory rejection。
這是 static DRAFT transparency snapshot，不是 clinical-trial registry、adverse-event reporting system、TFDA／IRB registry、complaint route、certification decision 或正式服務。
四種 classification
inconclusive
已有部分 evidence，但不足以回答指定問題或支持升級 maturity／claim。
missing
達成指定 claim 或 maturity 所需的 evidence，在目前 repo snapshot 中未找到。
out-of-scope
現有 evidence 有意只回答較窄問題，不能用來回答更廣的臨床、法規或服務 claim。

Publication and update rules
- 只記錄可追溯到 repo／approved evidence source 的內容；不得把推測寫成 observation。
- Missing evidence 只限制 claim／maturity，不代表介入無效或有害。
- Negative engineering finding 必須明示測試範圍、平台、資料型態與不可外推的邊界。
- 更改 classification、關閉 gap 或升級 maturity，需保留 source、具名 owner／reviewer、日期與 decision trail。
- Registry 是 read-only public information，不是 complaint、incident、adverse-event 或 PHI intake。
